Applicant Survey

If your preferred first name is different from the first name listed in University records, let us know!
RECOMMENDATION: We require one letter of recommendation for each applicant. Our preference is for recommenders who are familiar with your teaching or who can evaluate your potential as a teacher (for example, a professor who has taught you in a discussion course, who can evaluate how well you interact with others). Your recommender can be a professor here at the University of Chicago or someone from another institution -- select the person who can give us the best insight into your teaching. There is space on this form for two recommenders, because some applicants feel that we can get a better picture of their teaching experience from two different people. But please, please, please don't feel that this additional space implies the existence of a secret preference on our part for applications with two recommenders. This is not the case! We only need one.
Only one recommender is required. If you would like us to look at two recommendations, you may specify your secondary recommender here.
Please specify the title of your writing sample here.
Please upload your writing sample here; the maximum length is ten pages. Before uploading your sample, make sure you've filled out and attached the Writing Sample Cover Sheet included in our application packet. We accept .doc, .docx, and .rtf files.
The paper for you to comment on is included in our application packet. We accept .doc, .docx, and .rtf files.
